Welcome to the Parish!
The new data protection rules effective from May 2018 require that all adults (16+) must give individual consent to their data being collected.
Please find here a family registration form (one adult and children) and for any other adults in the family please complete the supplementary form (or single adult registration form) . 
If you have already completed a form but there have been changes to your personal data, please send us an updated form.
Completed forms can be returned by email to office@hendredcatholicparish.org.uk or by hand to Fr Andrew when at church.
